Thank you in advance to the GENIUS who figures this out!

I need to take the data captured by Smart Former on a Joomla 1.0.15 site to compose a custom URL that at the end will need to be executed.

Example:
This is the Base URL: Http://Sformercapture.com/ where I need to send my final data.

Below is the Base URL with the appended data that I need to create and execute after the user completed the form.

Http://Sformercapture.com/Name=Bob Marley&Address=100 Main St&PostalCode=90210&Phone=555-555-5555i ... helpme.com

I know this is simple for some of you guys, buy I’m a newbie who would really appreciate your help.
